ONLINE THERAPY and ONLINE COUNSELING
All Clients: Due to ongoing public health concerns, I am conducting online therapy and counseling only, via VSEE (HIPAA compliant) until further notice. The link to all video sessions is below.
Returning Clients: Please complete the new HIPAA & Consent to Treat form and email it to me prior to our next scheduled appointment.
New Clients: Please complete all of the paperwork in the blue box to the right and email to me at email@example.com (HIPAA compliant) prior to our first appointment.

Please complete all forms in the blue box to the right (for both online therapy and in-office therapy) and email them or bring them with you to your first therapy and counseling appointment.
FEES & BILLING
Planning for the required financial investment in therapy and counseling is an important part of the therapy process. Payment can be made with FSA, HSA, debit card, or credit card. For accounting reasons, I am unable to accept cash or checks.

All online and in-person therapy charges are run through SQUARE. I am listed as “MLO” on your statement for your own privacy and protection. You will NOT receive a receipt, as receipts run through SQUARE are not HIPAA compliant. If you have set your card account to notify you (or anyone else) of a transaction on your card, you will receive those alerts per your own settings. Please let me know if you have any questions at all.
I am not contracted with any insurance panels. Please contact your insurance company and ask how much they will reimburse you for a 90837 60-minute Telehealth or In-office therapy session in the Metro Atlanta area.
SIMPLER SUPERBILL PROCEDURE
- Email me at email@example.com with SUPERBILL REQUEST in the subject line.
- Include your full name, date of birth, dates and minutes of each session (60, 90, 120, 180) and the amount you paid for each session.
- I will send you a Superbill, along with an apology that it took longer than it should have.
2-DAY CANCELLATION POLICY
Please notify me at least 48 hours in advance of a cancelled or missed appointment to avoid being charged in full for my time. Email your cancellation to firstname.lastname@example.org. Cancellations are not accepted by text or voice mail. Thank you for your understanding.
