I schedule appointments by phone or by email and generally return calls and emails within 48 hours.

Please complete all of the forms in the blue box to the right and bring them with you to your first appointment.  Some of these forms will take time to complete.  Therefore, I encourage you to allow yourself sufficient time.

Please note that prioritizing the financial investment in your Therapy is an important part of your process.  Therefore, please let me know if you have any questions about this.

INSURANCE COMPANY COMMUNICATIONS – Helpful Hints

I am an Out-of-Network provider for all Insurance Plans.  Because of this, I recommend that you contact your insurance company directly for information about their reimbursement rates under your plan.  (The insurance code for a 60 Minute Outpatient Therapy appointment is 90837.)

Please tell them that you need the exact dollar amount of your direct reimbursement (for 90837 service) to calculate your out-of-pocket expense.  In addition, please ask them what their reimbursement turn-around time is so that you can budget accordingly.  Finally, I recommend documenting the details of each phone call including 1) the phone number you dialed, 2) the information provided, 3) the name of the person you speak with, 4) the date and time of the call, and 5) the call confirmation number.  It is ideal to keep a record of these calls.

CLAIMS AND REIMBURSEMENT

To submit a claim for reimbursement to your insurance company, please let me know that you need a Superbill/Invoice/StatementPlease allow two weeks from the time of your request.  Your Superbill will be emailed to you by my assistant.  Please be sure to check your Spam Folder for an email from jmaebabcock@mbaide.com.

CANCELLATION POLICY

To avoid paying in full for your cancelled appointment, please notify me at least 2 Full Business Days prior.  I will always respond and confirm receipt of your cancellation.  Therefore, if you do not hear back from me, I did not receive your cancellation notice.  In addition, Monday appointments should be cancelled by the Friday before.  Thank you for your understanding.
